What insurance companies do you accept?
Aetna, Blue Cross/ Blue Shield, Cigna, Humana, Medicare, and United Healthcare.
What are your hours?
Mondays: 9:30 am to 1:00 pm, Tuesdays: 9 am to 5 pm, Wednesdays: 8:30 am to 6:00 pm, Thursdays: 10:00 am to 4:00 pm, and Fridays 7:15 am to 12:15 pm.
What should I expect during my first visit?
You will be asked to complete some forms that allow us to provide better quality care for you. These forms can be filled out in the office or online. The nurse will then perform a quick assessment and you will then be shown to Dr. Cook’s office. Dr. Cook will review your medical history with you, answer any questions you may have and then show you to the exam room. Following your exam, you will again meet with Dr. Cook who will then discuss the findings of her exam with you and give you any recommendations she may have.
At what age should I have my first pap smear?
The American College of Obstetricians and Gynecologists recommend that we have our first pap smear at the age of 21 or when we first become sexually active- whichever one occurs first.
Where are you located?
We are at the intersection of the East West Connector and Cooper Lake Rd in Smyrna, Georgia.
What forms of birth control do you offer?
Dr. Cook offers oral contraceptives, the IUD- intrauterine device, depo provera, the vaginal ring, and tubal ligations that can either be done in the office or at the hospital.
What types of procedures do you perform in your office?
Essure tubal ligations, uterine ablation for abnormal uterine bleeding, colposcopies for abnormal pap smears, LEEP for removal of abnormal cervical cells, endometrial biopsies and vulvar biopsies.
Do you provide bioidentical hormone therapy?
Yes. Dr. Cook will write you a prescription that you can have filled at a compounding pharmacy of your choice that makes bioidentical hormones.
